Assignment of Benefits

This 1.5 hour session will give an overview of what an 'assignment of benefit' is and when it should be completed. We will review the following assignment of benefit forms:

• Agreement to Reimburse (form 2208)
• Assignment, Authorization & Direction (form 2209)
• Expected Compensation or Settlement Referral (form 2749)
• Canada Pension Plan (CPP) Consent to Deduction, Payment & Authorization to Communicate (form 1613)
• Old Age Security (OAS) Consent to Deduction, Payment & Authorization to Communicate (form 3010)
• Assignment of Workplace Safety & Insurance - WSIB (form 0840)
• Employment Insurance (form INS3113) - the group will go through this step-by-step
